Download the bas file then follow these instructions;
Got to Tools-Macro-Visual Basic Editor
The Visual Basic editor will open
Right click on Project1 and select Import- File, select the GTD Toolbar.bas file, it will create a module called GTD
Got to Tools-References 
and select the Microsoft Scripting Runtime option, press OK.
From the File menu select Save, then select Close and Return to.. Outlook
Got to Tools-Macro-Macros, run the GTD_Toolbar macro.
The GTD toolbar will be created.
When you open Outlook if prompted select the Enable Macros option.
